Creative Rehab accepts Personal Injury cases with the understanding that the patient is ultimately responsible for payment of services. As a service to the patient, we will bill the 3 rd party payor directly. Patients are required to submit documentation of medical insurance as a secondary method of securing payment at the start of care in order to verify benefits. Creative Rehab will place a lien against the 3 rd party payor to ensure consideration for reimbursement at the time of settlement. In the event a claim is denied, we will bill the medical insurance for payment. The patient remains responsible for in-network deductible and co-payments as applicable if the medical insurance is the responsible payor or for unpaid balances if insurance is not available.
