What to Expect in Physical Therapy

Creative Rehab strives to provide a relaxed, professional clinic in which to rehabilitate. Our philosophy of care is based on a strong belief in the partnering between the patient, physician and therapist in order to maximize function and the success of treatment. In order to be successful it is imperative that the patient participate fully with regular performance of the home program, regular attendance and open communication with the therapist. A strong relationship is pivotal to a positive therapeutic experience.

Before Therapy: When your physician determines that physical therapy is appropriate, a prescription will be written and either forwarded to our clinic or provided to the patient for follow-up. In the state of Illinois, a written prescription is required for physical therapy beyond the initial evaluation. It is the policy of Creative Rehab to require a written prescription prior to the initial consultation.


At the time of scheduling, we will obtain the patient's personal and insurance information in order to verify insurance benefits prior to the start of care. Creative Rehab provides Private Treatment Rooms

During Therapy: All appointments are scheduled one-on-one with a licensed physical therapist that will work with your for the duration of the rehab process. We provide individual private treatment rooms for confidential evaluation and treatment to ensure your privacy and comfort.

For the initial visit and follow-up, it is important that we are able to observe the area(s) to be treated. We recommend that loose fitting clothing, shorts/tank-tops or sportswear be either worn or available for changing into in order to optimize our ability to evaluate and treat effectively. The evaluation process will include a period of interview followed by assessment and initial treatment. We will begin our education process at the first visit and with each subsequent visit. Follow-up visits will be determined based on the evaluation, physician's orders and patient availability.

The treatment plan will be discussed with you for your input and goal development. Active participation and regular attendance are expected to achieve outlined goals.
